What you are consenting to
You are choosing to receive care from independent licensed providers through electronic means — structured questionnaires reviewed asynchronously, and live video visits where your state requires or your case needs one. Telehealth has benefits (speed, access, convenience) and limitations (no hands-on physical examination; technology can fail).
Your provider can say no
A provider may determine telehealth is not appropriate for your situation and decline to prescribe, recommend an in-person evaluation, or refer you elsewhere. That judgment is theirs alone and is part of safe care.
Your responsibilities
Provide accurate, complete health information; use medications only as directed; report side effects through check-ins or messages; complete follow-up labs when your protocol calls for them; and seek in-person or emergency care when your condition needs it.
Records and follow-up
Your telehealth interactions become part of your medical record on the platform. You may be asked to complete periodic lab work as a condition of continuing certain prescriptions — that is a patient-safety requirement, not an upsell.
Consent
By creating an account and checking the consent box, you acknowledge you have read and understood this notice and consent to receive care via telehealth. You may withdraw consent at any time by discontinuing use and notifying support.
